Mucomyst
Trade name: Mucomyst
Generic name: Acetylcysteine
Who: Patients with decreased renal function in preparation for IV contrast. (Patients who have overdosed on acetaminophen).
What: Renal protection against IV contrast. (Antidote to acetaminophen that inactivates intermediate metabolite of acetaminophen to protect the liver)
When: Before IV contrast: twice daily for two days, starting the day before the procedure. (For acetaminophen overdose, after loading dose give Q4hours for 17 doses).
Where: In hospital or home setting one day before procedure, (or acute care medication for emergent treatment of overdose)
Why: Given to prevent Radiocontrast-Induced Renal Dysfunction when IV contrast is needed. (Given to prevent or lessen hepatic injury following overdose of acetaminophen).
How: PO 600 mg twice daily for 2 days beginning the day before the procedue. (For Tylenol overdose: PO loading dose is 140 mg/kg, maintenance dose is 70 mg/kg. IV: Loading 150 mg/kg, maintenance 50-100 mg/kg).
